SUMMARY OF POSITION :

This position serves as an essential liaison between patient, family, physicians, community, and organizational resources to promote the coordination of a comprehensive treatment plan for the Family Birthing Center, NICU, and Pediatric population. Social Worker will collaborate with physicians, staff nurses, and other clinical personnel to determine patient care needs, readiness for discharge, and will communicate these needs with community based resources. This position plays a significant role in making connections and referrals to home health agencies and other social and governmental departments that would be the best resource for ensuring patient needs are met. Our goal is always to provide an exceptional experience while helping our patients and families get the resources they need to thrive.

MINIMUM QUALIFICATION(S):

Masters of Social Work degree required.

PREFERRED QUALIFICATION(S):

Previous experience with the FBC/NICU/Peds population and/or Children’s Youth Services.
