Job Title: Social Worker - MSW - Case Management for Adult Day

Type: Full Time, Monday-Friday 7:00AM-3:30pm

Location: Santa Ana, CA

Responsibilities:
-Evaluate clients’ psycho-social needs;

- Evaluate clients’ family/social situation to support a treatment plan;

-Utilize community resources to assist clients’ psycho-social needs;

-Provide referrals and admission to Adult Day Health Care program(s);

-Conduct in-home evaluations;

-Provide discharge planning;

-Document history, care plans and prepare reports as needed.

Experience Requirements:
-1-2 years of experience as a Social Worker.

- 1 year of experience working with elderly or disabled adults.

Education Requirements:
-Graduation from an accredited university or college with a Master's Degree in Social Work.

Qualifications:

-Current AHA-approved CPR card

-Skills in critical thinking, problem-solving, logic and reasoning, judgment, time management techniques and data entry;

-Ability to make improvements or take corrective action;

-Observe, receive, and otherwise obtain information from all relevant sources;

-Organize, plan, and prioritize work;

-Communicate with people outside the organization, represent the organization to patients, clients and the public;

-Deal calmly and effectively in high stress situations;

-Develop constructive and cooperative working relationships with others, and maintain them over time;

-Work within a team as well as independently;

-Speak, read, and write effectively as appropriate for the needs of the audience while demonstrating the proper use of grammar.
