Staff Nurse - RN - Home Care Admissions jobs in Kansas
Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Phoenix Hospice Admission Registered Nurses assess and determines hospice appropriateness of patients seeking end of life care. The admission RN should possess strong skills in physical assessment, diagnostic interpretation, communication, and multidisciplinary approach that encompasses the physical, but also the spiritual and psychosocial needs of the patient and their family. Admission nurses work closely with our intake coordinators, hospice consultants and medical directors to ensure a smooth transition from curative care to hospice care.
Pay Range: $65,000 - $90,000
Hours: Monday - Friday 10am-7pm or 11am - 8pmLocation: Topeka, KS

Responsibilities:
- Skilled Visits
- Day-to-day patient interaction
- Education on Health Conditions, such as CHF,CODP, Diabetes
- Wound Care
- Medication Management
- Changes dressings, bandages and contraptions as prescribed
- Documentation of patient’s condition and pointing out significant changes, to be reported to the physician
- Working alongside other nurses, facility staff, physicians, intake coordinators, and hospice consultants
Requirements
- Kansas or Compact Nursing License
- Strong organizational and self-management skills
- Valid driver’s license
