    UW Medical Center - Montlakehas an outstanding opportunity for aRegistered Nurse Heart Failure Nurse Coordinator to join our Cardiac Transplant team.Heart Failure Nurse Coordinators are specialized nurses who collaborate with a multi-disciplinary team consisting of heart failure / cardiac transplant / mechanical circulatory support trained cardiologists, cardiac surgeons, interventional cardiologists, other sub-specialty nurses, advanced practice providers, social workers, and administrative staff to coordinate, support, and optimize the medical care of this complex patient population. In addition to clinic and phone triage, the Heart Failure Nurse Coordinator is responsible for coordinating and managing medication titrations per provider orders (including home infusion of inotrope drips), central line dressing changes, and blood draw for labs.

    Heart Failure Nurse Coordinators also monitor the CardioMEMS HF System (remote pulmonary artery monitoring) for optimum heart failure medical management with the goal of reducing hospital readmissions and improving patient quality of life.

    SCHEDULE:
    100% day shift. This is a temporary position expected to last several months.

    RESPONSIBILITIES:
    • Provides expert nursing care to heart failure patients
    • Systematically assesses the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff
    • Ability to formulate, coordinate and revise an individualized plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness
    • Provides care for individual patients over the telephone using the nursing process
    • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors
    • Integrates patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and are tailored to the patient’s and family’s ability to comprehend and participate in care
    • Coordinates the care given by others to achieve efficient clinic operations
    • Assumes responsibility for continued professional growth and sharing expertise with other team members

    REQUIREMENTS:
    • BLS certification required
    • Minimum of 2 years of cardiac-related nursing experience
    • Washington State RN license required by start date
