Job Description:
The Certified Social Worker (CSW) utilizes clinical expertise to perform psychosocial assessment and develop/ implement a plan of care/treatment plan that provides extraordinary care for the patient while being sensitive to costs and resources. This position works collaboratively with patients, patients' family/significant other, healthcare providers, insurers, community resources, and other involved parties to identify patients' needs, broker services, coordinate care and transition plans, and advocate for patients. The CSW performs these duties under the direct supervision of an LCSW.Scope
1.Provides comprehensive intake examinations and behavioral health assessments, anddetermines accurate DSM-5/ICD-10 diagnoses based on clinical information.
2. Provides behavioral health services to individuals, couples, families and groups, using a variety of treatment approaches and tools.
3. Makes appropriate referrals for patients needing services outside the scope of the immediacy of primary care.
4. Maintains complete and timely clinical records on all clients.
5. Provides consultation to other staff members as requested and participates in staffing of cases.
6. Provides consultation and education services to individuals, agencies, institutions and groups as assigned.
7. Participates in staff training and attends staff meetings.
8. Responsible for the review of client files to assess dangerousness and violence evaluation of family violence referrals.
9. Responsible for making referrals to a variety of social services and medical resources as needed.
10. Participates in the screening process of new referrals to the department as needed.
11. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards.
12. Performs other duties as assigned.
Minimum Qualifications
Required
Masters degree in Social Work is required
Current BLS certification or ability to obtain within 3 months of hire is required
Two (2) years clinical experience in related field is required
Preferred
- Current LCSW in the state of practice; for the MSW candidate the ability to obtain SWLC within3 months of hire

This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.- and -Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.- and -Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items.
