**Job Purpose:**
As the Social Service Director, your primary role is to plan, organize, develop, and oversee the Social Services Department's overall operation. This involves ensuring that the medical, emotional, and social needs of residents are met on an individual basis, aligning with current standards, regulations, and facility policies.
**Delegation of Authority:**
In your capacity as the Director of Social Services, you will have the administrative authority, responsibility, and accountability required to execute your assigned duties.

**Major Duties and Responsibilities:**
**Administrative Functions:**
- Plan, develop, organize, and evaluate social service programs, ensuring compliance with regulations and professional standards.
- Keep current with federal and state regulations, recommending changes to policies and procedures.
- Develop policies and procedures for identifying residents' medical, social, and emotional needs.
- Participate in community planning related to the facility's interests and residents' services.
- Assist in discharge planning, developing social care plans and resident assessments.
- Conduct interviews with residents/families privately.
- Complete necessary administrative requirements, such as forms and reports, submitting them to the Administrator as required.
- Involve residents/families in planning social service programs.
- Assist in arranging transportation to other facilities when necessary.
- Refer residents/families to appropriate social service agencies when needed.
- Provide consultation to staff, community agencies, etc., to address residents' needs and problems through social service programs.
- Maintain a positive relationship with the medical profession and other health-related facilities and organizations.
- Coordinate social service activities with other departments.
- Work with facility consultants and implement recommended changes.
- Provide written and oral reports/recommendations to the Administrator regarding Social Services Department operations.
- Assist in standardizing work methods.
- Interpret department policies and procedures to employees, residents, visitors, and government agencies.
- Stay informed about economic conditions/situations and recommend adjustments to assure continued ability to provide daily social services.
- Review and develop a plan of correction for social service deficiencies identified during inspections.
- Maintain an ongoing Quality Assurance Program for the Social Services Department.
- Participate in survey inspections by authorized government agencies.
- Interview residents or family members to obtain social history.
- Ensure that progress notes are informative and descriptive of the services provided and residents' responses.
- Maintain a reference library of material for complying with standards and regulations.
- Involve residents/families in planning objectives and goals.
- Meet regularly with Administration, medical and nursing staff, and other related departments to plan social service programs and activities.
- Perform other duties as deemed necessary or directed by the Administrator.
**Personnel Functions:**
- Recommend to the Administrator the number and level of social services personnel to be employed.
- Maintain a positive working relationship with other department supervisors to ensure uninterrupted daily social services.
- Create and maintain a warm, positive, and calm environment throughout the facility.
**Staff Development:**
- Assist support services in developing, implementing, and conducting in-service training programs related to the Social Service Department.
- Attend and participate in continuing educational programs to stay informed about changes in the profession and maintain licensure.
**Safety and Sanitation:**
- Ensure that social service work areas are clean and sanitary.
- Follow Universal precautions.
**Equipment and Supply Functions:**
- Recommend equipment and supply needs to the Administrator.
- Place orders for equipment and supplies as necessary.
- Conduct periodic rounds to check equipment and ensure it is working properly.
- Ensure personnel operate social service equipment safely.
- Develop and implement procedures to use supplies efficiently and avoid waste.
**Care Plan and Assessment Functions:**
- Develop preliminary and comprehensive assessments of residents' social service needs.
- Create a written plan of care for each resident, identifying problems/needs and goals.
- Encourage resident participation in the development and review of their plan of care.
- Assist in scheduling care plans and assessments for presentation and discussion at committee meetings.
- Review nurses' notes to ensure care plans are being followed.
- Revise care plans quarterly or as necessary.
- Maintain a good rapport with all services involved in the care plan to ensure a team effort.
**Budget and Planning Functions:**
- Forecast department needs.
- Assist in preparing and planning the department's budget and submit it to the Administrator for review, recommendations, and approval.
- Make departmental adjustments to conform to the approved budget.
**Residents' Rights:**
- Maintain confidentiality of resident care information.
- Ensure social services personnel are knowledgeable about residents' rights and responsibilities.
- Review and report on resident complaints and grievances to the Administrator.
- Maintain a written record of resident complaints/grievances, actions taken, and current status.
**Miscellaneous:**
- Conduct weekly inspections to ensure quality control measures are maintained.
- Handle emergencies as they arise, rescheduling work assignments and schedules as necessary.
- Ensure appropriate protective clothing/devices are available for handling infectious waste/bodily fluids.
- Work with facility consultants and implement recommended changes.
**Social Services Director for Long Term Care Facility:**
- The social worker will work with residents in the nursing home, identifying psychosocial, mental, and emotional needs and providing, developing, or aiding in accessing services to meet those needs.
- A Bachelor's Degree in Social Work or a related field is preferred, or equivalent work experience in Long Term Care.
