Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. Summary:
We are growing and we are seeking a compassionate and organized Care Coordinator to join our team. The Care Coordinator will be responsible for providing the best judgment and patient education in a holistic approach in matters that affect true quality care of individuals, such as through case management, assessments, and providing needed resources. This position integrates health, psychological, physical, financial, and social care needs for members, ultimately preventing rehospitalization. The employee may also perform diverse duties for various Diversified Nurse Consultants programs.
Responsibilities:
- Conduct assessments of members needs and develop individualized care plans
- Communicate and collaborate with healthcare providers to ensure continuity of care
- Advocate for members to ensure they receive appropriate services and support
- Monitor and evaluate member progress and adjust care plans as needed
- Provide education and support to members
- Maintain accurate and up-to-date records of member care
- Stay current on healthcare trends and best practices
Qualifications:
- Experience in healthcare, customer service, social work or nursing preferred
- Excellent organizational and communication skills
- Ability to work independently and as part of a team
-Must comply with company credentialing requirements
-Must have a good working computer equipped with high-speed internet, and the ability to keep computer secure per HIPAA guidelines.
- Excellent problem-solving and critical thinking skills
- Proficiency in Microsoft Office and electronic medical record systems
