The Disease Management Case Nurse acts as a liaison between patient/family, employer and healthcare personnel to ensure necessary care is provided promptly and effectively. Coordinates the overall interdisciplinary plan of care for a patient in a disease management program, from admission to discharge. Being a Disease Management Case Nurse requires an associate's degree/bachelor's degree, and is licensed to practice nursing. Responsibilities include but are not limited to documenting case progress, identifying health risks, and reporting the findings of the case study at appropriate intervals.
