Job Description Job Description HOME CARE ASSISTANT IN HOME SERVICES PROGRAM Job Summary: The Home Care Assistant works under the direction of the RN Assessor and under the supervision of the Home Care Supervisor in their assigned area. This position assists seniors and/or disabled clients with tasks as outlined by the client’s current care plan. Duties and Responsibilities: Other Duties As Assigned By Supervisor And Management: Performs In-Home Services duties as outlined by the aide assignment and service plan assignment. Arrives and departs to each client’s home at the time designated on the schedule/aide assignment. Documentation of this is required showing the date, time, location, nature and scope of authorized services delivered.
