What You'll Do with Us as an Outreach Worker: Provide health education and risk reduction activities to persons at risk for HIV and HCV. The Outreach Worker is part of a team who engages in on-site outreach programming at the Milford site as well as street-based outreach programming. BCCS Outreach programs offer an array of support and resources, including anonymous needle exchange services, free HIV and HCV testing

Overview: Train and consult with pregnant women to help them achieve adequate prenatal and post-natal care as well as encourage proper care of their infants through age 2 through a home visiting approach. Responsibilities: Develop knowledge competencies regarding evidence-based best practices for maternal child health and home visiting as directed by supervisor. Visits in home and community settings with pregnant and post-partum women and/or fathers regularly from the first confirmation of pregnancy until the infants second birthday, utilizing approved curriculums and evidence-based best practices.
