Provide transitional intensive case management services on-site at the New York City Department of Homeless Services Bellevue Assessment Shelter, at other shelters if needed and in the community for up to 70 clients per year for a period of 3 to 6 months.

Attend inter-agency case coordination meeting, coalitions, regional advocacy groups and boards as appropriate in benefiting the participants served and support community cooperation in addressing areas of need.

Collaborate with Reception & Placement Case Managers, Refugee Family Services & Connections Case Managers, and Cultural Navigator Volunteer Teams to ensure clients understand how to navigate health and mental health systems.
