Outreach Caseworker
Location: Weymouth
Contract type: Fixed term for 9 months with possibility of an extension
Hours: 35 per week
Salary: Up to £30,000 per annum £300 flexi working allowance

Are you a highly experienced Outreach Caseworker in the Weymouth area who would love to work on an innovative NHS programme (High Intensity Use (HIU) Programme)? The aim of the programme is to change the perceptions of people who access health care more than expected. The team help to prevent people from reaching crisis point, empowering them to better self-manage and connect with their surroundings.

Within this role there may be a need to visit locations which have limited access by public transport. Therefore you must have a UK driving licence and access to a vehicle or be eligible for transportation via the Access to Work Scheme.
