Summary The Advanced Practice Registered Nurse and the Physician's Assistant provide primary medical care, and preventive health care services to patients throughout the lifespan. Functions in one or more Concord Hospital Medical Group (CHMG) settings including office, patient home, nursing facility, or inpatient. Patient care is provided under the supervision of a CHMG physician for the Physician's Assistant, and in collaboration with a CHMG physician for the Advanced Practice Registered Nurse. Education Completion of Master's degree in Advanced Practice Nursing or Physician's Assistant. Certification, Registration & Licensure Licensure required: NH License as APRN or Physician's Assistant, Current DEA License Experience Prior primary care and/or site specific experience may be required based on the practice setting. New graduates may be hired. Responsibilities Obtains patient health history, performs physical examination, and develops a relevant differential diagnosis. Orders, interprets, and follows up on diagnostic tests. Diagnoses acute and chronic medical problems within the scope of practice, or formulates a comprehensive differential diagnosis. Prescribes and manages medications within the practice standards. Provides individualized patient education related to preventive care according to guidelines.
