Qualifications:
Bachelor’s degree in Nursing is optional
Must have an RN license
A minimum of two (2) years of experience as a registered nurse, home health and hospice experience preferred
Current driver's license is preferred
Current CPR and BCLS certification required
Knowledge of Medicare, Medicaid and insurance benefits is a plus
Effective communication skills
Great administration and documentation skills
Ability to exercise initiative and independent judgment
The main responsibilities of this position include, but are not limited to:
Understands and adheres to the agency's policies and procedures.
Initiate the admission process of the patient.
Complete an initial, comprehensive, and ongoing assessment of the patient and family to establish hospice needs, including a complete physical assessment and medical history; determine care plan needs and initiate the plan of care.
Complete, maintain, and submit all necessary paperwork, including obtaining consents and benefit elections, as well as accurate clinical documentation and relevant notes regarding the patient’s condition and care provided.
Explain your agency hospice philosophy to family members.
Reviews the DNR (do not resuscitate) document.
Ensures that arrangements for equipment and other necessary items and services are available. Orders medical supplies, as necessary.
Provide a safe environment for the patient by checking living space with safety hazards.
Educates family or the caregiver on how to administer medications, indications, and its side effects.
Communicates with other clinical team members to coordinate the care plan.
Relate and perform work assignments in a calm and professional manner especially during times of crisis.
Obtains/receives physicians orders as required.
Counsels the patient and the family in meeting nursing and related needs.
Observes confidentiality and safeguards all patient information.
Work effectively on a minor supervision.
Takes on-call duty nights, weekends, and holidays as assigned.
