QUALIFICATIONS
1. Current licensure as a Registered Nurse in Illinois and CPR certified.
2. At least one (1) year of clinical nursing experience, preferably in rehabilitation or medical surgical nursing. Home care experience preferred. (This clinical experience can be during nursing school for the new RNs and LPNs)
3. Interpersonal, communication, documentation and organizational skills necessary to complete job responsibilities.
4. Must have vehicle, current driver's license and appropriate automobile insurance.
5. Ability to make home visits and provide skilled nursing assessments and care in the home.
6. An individual may not pose a "direct threat" to the health or safety of the individual or others in the workplace or in performance of job
JOB DUTIES: (include the following but not limited to)
1. Makes home visits, performs physical assessments, identifies patient's needs and develops plan of treatment in collaboration with patient/family/advocate, Physician, and Supervisor, as assigned and documents appropriately
2. Assesses need for additional services and equipment on admission and on an ongoing basis and informs Supervisor on a timely basis.
3. Assesses home environment and makes recommendations that will enhance patient care, promote safety and prevent adverse effects.
4. Revises and updates plan of treatment in collaboration with patient/CG/physician as needs change and as goals are met and communicates changes to appropriate staff and documents same promptly.
5. Coordinates plans for patient care outcomes through formal and informal case conferences with other team members to provide appropriate, effective and efficient patient care and discharge planning and documents appropriately.
6. Performs assessments/reassessments of patients at admission, every 90 days, and as patient’s experience significant changes in condition. Completes documentation accurately and timely and consults with other health care providers and caregivers, as needed and documents same legibly and submits promptly.
7. Provides clear, written and verbal instructions to Aides and LPNs based on their legal scope of practice, updates written instructions in a timely manner and documents appropriately.
8. Informs physician promptly of changes in patient’s condition in accordance with physician’s orders and Agency guidelines and documents communications with physician appropriately.
9. Supervises LPNs to evaluate competency, in accordance with state regulations, or more frequently if necessary, provides appropriate instructions to LPN and documents same.
10. Conducts Home Nursing Aide supervisory visits as assigned and at least every 60 days, and as indicated and evaluates the aide's competency, the client service plan/assignment and provides appropriate instructions to the aide and documents same
11. Initiates appropriate preventive, rehabilitative, therapeutic, comfort, and pain interventions and provides instructions to patients/caregivers, and staff in a timely manner
12. Updates patients’ clinical records including Medication Profiles, Plan of Treatment, Case Conferences, and re-assessment documentation, as required on a timely basis
13. Completes clear, concise and accurate clinical notes, which document skilled services and other, required documentation appropriately and submits on a timely basis. Utilizes only approved abbreviations
