** 1-2 weeks of in office training. Must be from the Corpus Christi, TX -or- McAllen, TX 78501 area**
Job Responsibilities / Description:
- Daily responsibilities:
- Assist with member tasks – follow up with members regarding coordination of care
- Follow up with providers and doctors’ offices.
- Performance Expectations/Metrics:
- Provided a monthly caseload (20 per month)
- Required to follow up with members on a weekly basis
- What previous job titles or background work will in this role?
- Home health experience
- Hospital experience.
HOURS: Monday to Friday - 8:00AM - 5:00PM
PAY: $35-$45 hourly (determined on experience)
Required Education & Experience
- At least 1 year of Case Management experience
- Basic computer skills (Microsoft Word, Excel)
- RN license, Associate’s Degree
- Microsoft Word, Excel, computer navigation.
Preferred Experience:
- Highly Preferred (but not required): Bilingual in Spanish
- RUG certification
- TruCare experience
- Prior home health care experience
- BSN Degree highly preferred
