Job Summary:

• Provides social work services to residents and families.
• The social worker assists the physician and other team members in understanding the significant social and emotional factors related to the health problems of the patient.
• The social worker participates in the development of the plan of care, prepares clinicals and progress notes, works with family, uses appropriate community resources, participates in discharge planning.

Qualifications:

• Master's or Doctoral degree from a school of social work accredited by the council on Social Work Education.
• Home Care experience desirable
• New York State Drivers License or access to transportation.
