Position Summary
The Manager, MCH and Women’s Health and Community
Integration will oversee maternal and child health care management, outreach,
and lactation program services for pregnant and parenting patients. This role
is responsible for working with staff to ensure the provision of quality care
management and community engagement for women, infant and children.
The Manager builds outstanding integration of care
management services with clinical services. Core Job Responsibilities:
• Responsible for conducting regular team meetings to
provide group training and continuing education; address general care
management concerns or needs
• Interface with funding agencies
• Integrate care management and other MCH programs into
clinical services
• Promote continuous quality improvement
• Lead the development of care management programming and
documentation within the electronic health record and/or funder-based data
collection software
• Develop reports for funders
• Develop and assist with in-service training for MCH staff
• Assist with planning and program sustainability
• Manage direct reports; communicating expectations,
providing coaching and feedback, monitoring and managing performance and
providing development opportunities.
• Conducting regular meetings to assess progress in
achieving goals, assess management of individually assigned caseloads, address
areas of concerned identified by quality assurance related to case notes and
care plan content etc.
Perform other duties as assigned Requirements/Preferences
• Bachelor’s degree (required) Masters’ Degree (preferred)
in nursing, health administration, social work, or public health
• Minimum of three (3) years paid professional experience in
community based primary health care delivery
• Two (2) or more years’ experience managing direct reports
preferred
• Minimum of one (1) year of project management experience;
demonstrated skill in project management
• Experience in maternal and child health preferred
• Experience with MCH data collection preferred
