Access TLC is seeking dedicated and compassionate Care Coordinators to join our team. The Care Coordinator has direct responsibility for an assigned caseload of patients and provides case management and transition coordination according to requirements of the California Community Transitions (CCT) program.
The goal of the program is to facilitate a safe transition of Medi-Cal eligible persons from a nursing facility to a home and community-based setting. Patients receive ongoing case management post transition to ensure the health and safety of the CCT participant in the community.
This position involves a combination of field based visits to patients and facilities, as well as remote work. Field based visit locations may require up to 2 hours of travel time. Travel time and mileage is reimbursed. Orientation training will be at our office in Moorpark or San Fernando.
Position is full-time and work hours are 8:30am-5:30pm Monday through Friday.
Job Duties/Responsibilities:
· Arrange and schedule in-person visits to conduct assessment of patients’ needs.
· Develop individual care plans in collaboration with the patient and care team.
· Arranging for services as determined necessary by the individual assessment. Services may include: pre-transition coordination, home set-up, habilitation and family and informal caregiver training.
· Provide monthly visits to monitor the patient to establish ongoing eligibility for the program, to monitor safety and services identified in the Individual Service Plan and address concerns of the patient.
· Serve as the primary point of contact for patients and their families on CCT services per assigned caseload. Educate patients and families about CCT services.
· Document assessments and patient interactions in the electronic medical record system and according to agency guidelines.
Job Requirements:
· Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology.
· One year of related work experience. Intern or volunteer experience is also applicable.
· Must have a California Driver’s License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements. Drivers must be licensed for at least 3 years.
· Bi-Lingual is a plus!
