Hospice Registered Nurse Case Manager, 9 AM-5 PM Mon-Fri.

What You Will Do:
RN Case Managers visit patients in their homes: private residences, assisted living communities and skilled nursing facilities. You'll make visits for assessment, symptom resolution, education, deaths, and admissions as needed. Use the skills you really went to nursing school for - education, assessment and disease management.
Qualifications:
- Excellent and clear communication, organizational, problem-solving, and multi-tasking skills.
- Current Registered Nurse (RN) licensure, in good standing, in the state of Illinois with at least 1 year of experience under license.
- CPR/BLS provider (current).
- Ability to collaborate with an interdisciplinary team, physicians, and allied health professionals.
- Work comfortably and closely with patients, their caregivers and families facing end of life issues.
- Good time management skills in order to see patients, update family members, attend meetings and complete documentation
- 1:1 on the job training for our web-based EMR, patient visits and charting.
- Reliable transportation available on all work days, valid driver's license, current insurance for vehicle.
