ACLD’s Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment.
In this position, you will:
- Provide on-site job coaching for designated caseload
- Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation.
- Conduct observational assessments on site in order to assist the person supported in learning the tasks of the job
- Travel train on the public transportation system and/or roadways
- Maintain case files for each person supported.
- Prepare all documentation for ACCES and OPWDD on a monthly basis
- Act as a liaison between the person supported, family and employer
- Attend all staff meetings, Life Plans, conferences and annual training requirements
- Provide overall assistance as assigned
We require:
- High School Diploma or equivalent; Associate’s Degree in Psychology, Sociology, Education or related field preferred.
- Minimum of one year experience providing vocational or pre-vocational services to supported persons preferred
- Complete (within first year of employment) twenty-four (24) hours of OPWDD’s Innovations in Employment Supports training.
- Clean, valid New York State driver’s license
