Since 1957, we’ve assisted a wide array of populations throughout Southwestern Pennsylvania, focusing on those generally underserved by traditional services including senior citizens, veterans, individuals with physical and/or intellectual disabilities, individuals on the autism spectrum, and youths who have aged out of foster care.
To further our mission we are looking for a part time Service Coordinator.
Position Overview:
Links residents to supportive/health services that will enable the resident to achieve/maintain his/her maximum level of self-sufficiency in independent living at our Quemahoning Towers location.

Links residents to supportive/health services that will enable the resident to achieve/maintain his/her maximum level of self-sufficiency in independent living at our Quemahoning Towers location.
Job Responsibilities:
- Performs an “in-home” general needs assessment for each resident on an annual basis
- Develops/implements general case management plan for each resident based on the needs assessment
- Follows-up with residents where appropriate on monthly basis or on the needs of the individual
- Maintains an active file for each resident containing all information on social service needs
- Works cooperatively with the property manager to address related issues
- Performs resident services surveys of all tenants annually and develops programs responding to unmet needs
- Works with the resident council to develop activities/programs beneficial to all residents
- Establishes links with social service providers/volunteer/support groups
- Handles all reporting responsibilities required by the HUD contract
- High school diploma or equivalent required. Bachelor’s degree in social work or degree in Gerontology, Psychology or Counseling preferred.
- Training in aging, elder services or disability services
- Two years experience in social serve delivery with senior citizens and non-elderly disabled
- Demonstrated working knowledge of supportive services for senior citizens and non-elderly disabled in the area
- Demonstrated ability to advocate, organize, problem solve and provide results for the elderly and disabled
