Job Summary:
The Program Manager provides oversight and supervision of community-based residential homes and independent apartments serving individuals with physical and mental health disabilities.
The Program Manager is responsible for programs, services, staff, and physical sites and ensures continuous compliance to all federal and state regulations.
The Program Manager provides leadership to the staff teams at each site and serves as a liaison between support team members and community agencies while ensuring compliance with all county and state regulations. This position is based out of our Bloomington office and reports to the CEO. Must be willing to travel to Minneapolis, Bloomington, Richfield, and St. Paul.
The schedule must be flexible to accommodate various facility, personnel, and organizational needs.
Job Responsibilities:
Programming:
- Complete all required and requested resident reports, ensuring that they are in compliance with licensing requirements, and also that they meet the expectations of the agency and resident teams.
- Implement resident programming that is person-centered and focused on safety, independence, and community integration.
- Makes recommendations about the type and amount of home and community-based services and in consultation with the parents or legal representatives coordinates the startup, delivery, and continuing schedule of all home and community-based services. Maintains and summarizes data for reports on the outcomes of services for the case manager and support team members on a requested basis.
- Be available to staff for telephone consultations.
- Monitors site budgets and works with the office manager to ensure the program is within the monthly budget.
- Instruction and assistance to direct support staff implementing the coordinated service and support plan and the service outcomes, including direct observation of service delivery sufficient to assess staff competency; and
- Evaluation of the effectiveness of service delivery, methodologies, and progress on the person's outcomes based on the measurable and observable criteria for identifying when the desired outcome has been achieved according to the requirements in section 245D.07.
- Maintains and performs periodic audits of individuals served case files to ensure accurate documentation of program-related delivery of services.
- Maintains record-keeping systems, recommends and executes correction plans resulting from citations.
- Maintains professional and influential relationships with support team members and consultants; provides effective recommendations to resolve dissatisfaction with contracted services.
- Coordinates appointments for all health or consultant-related services including nursing, specialist services, physicians, psychologists, and all other consultants ensuring resolution of outstanding issues.
- Manages and collects rental payments from residents of each site
- Coordinates staff scheduling for and coordinates the delivery of all authorized methods to achieve service outcomes, including those related to supervision and monitoring, behavior management, activities of daily living, mobility, and use of senses, use of the community, health and emergency intervention,
- Monitors the program site including, maintenance, cleaning, utilities, adequate furniture and furnishings, and neighborhood relations.
Work with HR:
- Determine appropriate staffing levels and hours for each assigned site with the site supervisors.
- Ensure the Program Supervisor is scheduling appropriate coverage at each site and monitors overtime.
- Work cooperatively with Human Resources regarding performance, reliability, hiring needs, and recommendations.
- Assist with verification of timesheets and payroll issues as necessary.
- Directly supervise Site Supervisors, Direct Support Professionals and provide support and supervision regularly.
- Provide ongoing staff supervision regarding resident interaction, ensuring that prescribed resident interventions and Individual Abuse Prevention Plans are followed.
- Mediate intra-staff conflicts.
- Complete performance reviews for direct reports. (Office Manager, Site Supervisors, DSP’s, Nurse)
- Provide training to Site Supervisors as well as to DSPs as needed.
- Provide ongoing instruction and training to staff regarding resident program plans, supervision requirements, symptom monitoring, and any other applicable resident information.
Job Qualifications:
- Bachelor of Science degree in a field related to human services and one year of full-time experience in working direct care with individuals with disabilities or Mental health issues.
- Associates Degree or Diploma in a field related to human services from an accredited post-secondary institution and 2 years (4,160 hours) of full-time experience in working direct care with individuals with disabilities or Mental health issues.
- Four years (8,320 hours) of experience in working direct care with individuals with disabilities or Mental health issues under the supervision of a staff person who meets the previously listed qualifications.
MUST HAVE one year of supervisory level experience in a program/service providing direct support services to persons with disabilities or persons age 65 and older.
Job Requirements:
- Valid driver’s license
- Have 3 year of 245D experience
- Must not be disqualified by appropriate criminal background studies.
- Complete all orientation and training needs as written in the policy.
- Possess good oral and written communication skills and organizational skills.
- Basic knowledge of Microsoft Office, Gmail, and Chrome.
- Must be tech-savvy and open-minded
- Experience or training in hiring and coordinating staff, supervision, and program/service plan development.
Working Conditions:
The Program Manager position is subject to flexible hours. This position will travel regularly into the community and work in a variety of settings both inside and outside the site and throughout the community. The Program Manager is subject to frequent interruptions, crisis management and imposed deadlines. This person is subject to problem solving activities and has regular contact with individuals, family members, friends, federal, state or county regulatory personnel, other provider personnel, and the general public, who may be hostile or emotionally upset. This position may be subject to verbal aggressive individuals and may be exposed to infectious diseases, odor, and dust.
