RN/LPN

We are currently seeking an RN/LPN with excellent communication skills to assist in quaterly client assessment. He/she will be responsible for assisting with

• perform quaterly visits to assess client well being
• report changes in clients needs to DON.
• place weekly calls to clients to check on their well being
• Communicate with family members or caregivers to keep them informed about patient's condition.
• Make sure that the patient has regular doctor's appointments.
• perform initial assessment with new clients, recertification, readmission
• prior medicaid LTSS experience will be a plus
• biweekly calls to clients to check on their needs

Job Type: Full-time

Pay: $20.00 - $30.00 per hour

Expected hours: 40 per week
