Conduct thorough patient assessments through interviews, chart reviews, physical exams, and considering various factors like illness, risk factors, family history, psychosocial context, and cultural aspects.
Order and conduct relevant diagnostic tests and screenings, seeking input when needed for a thorough evaluation.
Put all the data together to reach a diagnosis and develop a comprehensive treatment plan, including identifying potential health risks.
Carry out treatment plans, follow guidelines and regulations, prescribe medications, and take care of minor lacerations.
Empower patients and families through education on medication regimens, treatments, and preventive measures, connecting them with community resources for ongoing support.
Share important case management information with colleagues and community organizations.
Keep accurate medical records, updating patient charts with test results, diagnoses, medications, and treatments.
Contribute to peer and chart reviews, staff education, development of clinical guidelines, and continuous education to ensure compliance with standards and quality protocols.
Offer expert consultation to the clinical team in your specialized area, always upholding patient confidentiality and advocating for them.
Collaborate with physicians to manage acute and long-term medical needs, ensuring continuous care through effective communication and triaging of patient calls and emails.
Foster collaboration with multidisciplinary team members by making appropriate referrals and maintaining effective communication with internal and external stakeholders.
Qualifications:
Required: BSN (Bachelor of Science in Nursing), Preferred: MSN (Master of Science in Nursing)
Completion of advanced practitioner training and certification from an approved nurse practitioner program, either in a general or specialized field.
Minimum of three years of professional nursing experience in a clinic setting, with an additional two years of nurse practitioner experience.
Current Indiana state registered nurse license.
Current Indiana state license as an advanced registered nurse practitioner with prescriptive authority.
Current CPR certification.
