JOB DESCRIPTION:
Summary/Objective
All-at-Home Health Care is a full-service home care agency dedicated to providing high-quality home care within the home environment through clinical and service excellence helping our patients maintain their independence and remain safely in their homes.
RN will be called upon on a per needed basis to provide oversight and orientation to the Direct Care Workers upon the start of care and either every 60 days, quarterly or annual reviews. In partnership with the Service Coordinator, RN will help maintain program regulatory compliance by developing and/or signing on Care Plans and HHA Assignments based on Elderly Agency’s HHA communication form or already developed Care Plans. RN will also be responsible to do quarterly supervisions to ensure that Care Plans are being followed by Direct Care Workers.
What we are looking for:
We are currently hiring for an RN to work part-time on weekdays in the following areas: Peabody, Salem, and Beverly. Must be bilingual in Spanish and English.
Responsibilities
- Review, sign, and orient worker on Care Plan for Personal Care services
- Develop and orient worker on HHA assignment for HHA services
- Provide quarterly worker field supervision and more as determined necessary
- Revise and orient worker on HHA every 60 days
- Revise, Sign, and orient worker on PC and Behavioral Care Plans annually
- Partner with ASAP Case Managers to understand expectations and evolving needs and respond with program development
- Promote safe, high-quality care
- Foster collaborative relationships with community-based providers and agencies to facilitate care to elders.
- Serve as a role model and mentor.
- Offer patient and family/caregiver resource education as requested
- Acts as a resource on questions related to ASAP relationships and access to services
- Document patient visits according to policy
Qualifications
Education: Graduate of an accredited RN program
License: Licensed as an RN by the Commonwealth of Massachusetts Board of Registration of Nursing. Unrestricted license to practice in the state.
1 year of field supervisor experience
Bilingual: English/Spanish is required
Local Travel is required

Must clear CORI background check
