Job Description
Care Coordinator
Care Coordinators are part of the Addiction Response Team and are responsible for working in collaboration with local providers and community agencies to ensure that individuals with Substance Use Disorders are successfully connected with resources to aid in their long-term health, treatment, and recovery goals. Care Coordinators will aid individuals in meeting their goals toward ongoing recovery through education, outreach, connectivity to medication-assisted treatment (MAT), and other treatment and recovery services. Care Coordinators maintain frequent communication with partner organizations to provide updates, identify barriers, and avoid duplication of services.

Care Coordinator

Knowledge / Skills / Abilities
- Experience working with and ability to build trusted relationships with persons with Substance Use Disorder, especially opioid use disorder, alcohol use disorder, and stimulant use disorder.
- Cultural Humility / Ability to effectively engage with different populations including People of Color, LGBTQIA , Disability, Veteran, Low/No income and/or experiencing homelessness, People with criminal justice involvement, and people who use drugs, people with mental health conditions, people with complex medical conditions and more.
- Critical Thinking and Problem-Solving Skills.

- Strong decision-making skills
- Strong verbal and written communication skills in-person, phone and email.
- Ability to maintain non-reactive and calm when engaging with individuals who may or may not be intoxicated, belligerent, or have a history of behavioral health issues
- Ability to adjust priorities quickly as circumstances and needs dictate for member success.
- Computer skills including the navigation of multiple Electronic Medical Record systems and data collection tools as determined by organizational needs
- Experience and/or ability to partner with professionals in local and regional health.
- Experience working with Medicaid and Medicare
- Bilingual preferred
- Ability to work independently and in a busy office environment that may contain interruptions to work due to walk-ins, member needs, supervisors request, phone conversations, etc.
- Reliable transportation
- HS Graduate or GED required
- Valid Colorado Driver License and Insurance required

- Enrollment of persons using appropriate assessment tools & completion of GPRA survey at enrollment and then at regular 6-month follow-up.
- Support persons enrolled in services to continue engagement with both medical and behavioral health treatment plans and assist persons in scheduling appointments, follow-up care, referrals, medication refills, etc.
- Provide appropriate health needs and clinic/non-clinical assessment, identify risk factors, and develop health plan and support with the integration of treatment and recovery services including Medication-Assisted Treatment.
- Monitor members according to guidelines set by NCHA Policy and Procedure under the Addiction Response Team guidelines
- Participate in telephone contacts, office/hospital visits, and other means of connecting with members to reach identified goals
- Perform health needs assessments and identify risk factors to develop a health care plan
- Assist members in scheduling appointments, follow-up care, referrals, medication refills, etc.
- Attend member appointments with providers and community resources as needed.
- Participate in health promotion and health education activities for members as identified in health care plan.
- Work in collaboration, communication, and coordination among all responsible parties of an individual member’s multidisciplinary team to support achieving goals and reduce duplication of services.
- Optimize member and family independence through education, community resources, and support
- Review appropriate cost-effective care and decrease duplication of services for members
- Understand, communicate, and facilitate on behalf of patients with regard to complaints, grievances, and appeal processes
- Regular case review with Supervisors and Trainers
- Strict confidentiality of member records and communications
- Completion of Certified Addiction Technician Classes and attend supervision as required.
- Safely transport members to and from MAT appointments in NCHA company vehicle
- Provide assistance as designated by the NCHA Organizational and Addiction Response Team Leadership to support the overall goals of NCHA
- Provide input into development of policies and procedures
- Support the mission and vision of NCHA
- Compliance with NCHA procedures
- Compliance with policies of other community agencies when working at their facilities
- Attend Internal and external meetings and completion of in person and virtual training as identified
- Participation in ongoing performance improvement activities
