To provide individuals with life enrichment opportunities and alternatives, including recreational, academic, social, and therapeutic services that enable them to function at their optimum potential with all the rights afforded them as full citizens.

The Direct Support Professional (DSP) will assist individuals in community integrated settings, including volunteer opportunities, and social/recreational activities.

ESSENTIAL FUNCTIONS will include, but not be limited too:

  • Actively assists individuals, providing reinforcement when appropriate, in order to increase independence.
  • Develops and facilitates activities that encompass social development, and that provide opportunities for growth based on personal strengths and goals identified in individual treatment plans.
  • Assists in updating data collection sheets to match individual goals with treatment plans.
  • Works to gather information and ideas for activity development, as well as for training needs.
  • Completes community-based programming with consumers as scheduled, including transportation, and records data on standard data sheets, in order to log progress or toward a goal.
  • Assists persons served with feeding, toileting and/or changing adult supports, as appropriate, following the individual’s plan of service.
  • Prepares and passes medication, as appropriate, and records medication delivery in the medication log.
  • Completes an annual assessment, attends each individual Person Centered Planning meeting as requested by the person and/or family/guardian and actively participates in goal formation process.
  • Completes reports as necessary, including incident reports, seizure reports, progress notes, and vehicle forms.
  • Attends, actively participates in, and completes scheduled training and in-services.
  • Attends required meetings.
  • Adheres to agency safety guidelines and procedures, maintains a safe working environment and completes light housekeeping duties including vehicle maintenance in order to ensure a clean, safe and healthy environment for individuals and co-workers.
  • Interacts with co-workers, individuals, supervisors, funding source, parents/guardians, community members and visitors in a professional manner.
  • Performs all other duties as assigned by the Supervisor and/or his or her designee.

Our Direct Support Professional (DSP), must be knowledgeable about and actively support culturally competent, recovery-based practices. Must be knowledgeable about person-centered planning as a shared decision making process with the individual, who defines his/her own life goals and is assisted in developing a unique path toward those goals. And must be knowledgeable of trauma informed culture to aid individuals in their recovery process emphasizing physical and emotional safety, choice, empowerment, and trustworthiness for individuals.

Physical Demands and Qualifications:

  • Flexible to work any shift and any location
  • Able to read, write, and effectively verbalize/articulate information.
  • Able to drive or have other transportation means to multiple sites within the service delivery area.
  • Able to lift a minimum of 25 lbs.
  • High school diploma or GED
  • Minimum of 1 year experience working in the human services field
  • Valid Driver’s License with an acceptable driving record.
  • Successful completion of the post-offer process including criminal history check and professional references.

Job Types: Full-time, Part-time

Pay: From $15.70 per hour
