What are the responsibilities and job description for the Health Nurse: LPN position at American Hearts & Home Llc?
We are looking for individuals who have a sincere desire to help others. Our mission is to serve adults with intellectual/ developmental disabilities, by providing specialized services including assisting with ADLs, tracking goals, community outings, etc. Supports are individually planned and tailored to meet the specific needs of the individuals we serve. This may include working at multiple homes, locations, and departments.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Adheres to assigned schedules and reports to duty for the hours assigned and implements planned schedules and activities
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Observe patients under treatment to identify progress, side-effects of medications etc.
- Supports in activities and companionship services for daily living, such as medication assistance, cooking, bathing, dressing, grooming, feeding, toileting, transferring, and other similar tasks
- Accompanies and transports to participate in visits for recreation, medical, and dental care appointments, therapies, personal shopping, and other community activities
- Implement ISP goals
- Assists in creating a physical environment that lends to progress including house cleaning and laundry.
- Other duties as assigned
Required Qualifications :
The successful candidate must:
- Be at least 21 years of age
- Have a high school diploma or GED (proof will be required)
- Graduate of an approved professional nursing program
- Have dependable transportation, a valid driver’s license, and valid car insurance
- Be able to satisfactorily pass a criminal background check
- Provide a clean 7-year driving record, with no suspensions due to moving violations in the past 3 years
- Must obtain a TB test
- Current CPR/First Aid certification
- Current physical
- Committed to health & safety
- Well-organized and reliable
- One year of experience working in the home health environment
- Positive, compassionate attitude, able to handle stress
Education:
- High school / GED (Required)
- Current LPN licensure without restrictions or provisions in the state of GA (Required)
