Please note, the per-diem rate of pay: $30-$35 per hour
The hospice aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity, and safety of a patient's care.
DUTIES & RESPONSIBILITIES
- Assist patients with activities of daily living, such as bathing, grooming, and dressing.
- Provide emotional support to patients and their families.
- Observe and report any changes in the patient's condition to the RN/ Case Manager/ and/or Director of Patient Care Services.
- Assist with light housekeeping duties, such as laundry and meal preparation.
- Assist with ambulation and transfers.
- Provide companionship to the patient and family members.
- Maintain a clean and safe environment for the patient.
- Ensuring respect of patients personal health information as required by HIPAA
- Acting and working with respect to the other caregivers or individuals living with the patient and their personal matters while ensuring patient advocacy.
- Checking on patient’s supplies to report to the agency any needed replenishment and refills.
Qualifications/ Position Requirements
- Current, unrestricted certificate as a Home Health Aide in the State of California
- Complies with accepted professional standards and practice
- Demonstrates good verbal and written communication in English, and organization skills
- Comfortable with any cloud technology and experience with Electronic Medical Record software
- Active CPR certification and verification of negative TB skin test or clear chest Xray within the last 12 months
- Must be a licensed CA driver with access to an automobile that is insured in accordance with state/or organization requirements and is in good working order for self-transportation to all work shifts
- Home Health or Hospice experience preferred.
