ABOUT THE AMERICARES FREE CLINICS:
The mission of the Americares Free Clinics is to provide quality health care to low-income, uninsured patients in a setting where all individuals are treated with dignity and respect. Americares opened its first clinic in 1994 to expand access to care for the working poor. Over the years our program has grown to become the largest free clinic network in the state with four locations serving 3,000 patients annually. Today, our clinics in Bridgeport, Danbury, Norwalk and Stamford are the primary source of free medical care for the uninsured in Fairfield County.
SUMMARY:
The Patient Care Coordinator is responsible for assisting with patient scheduling, patient contact, and primary referrals made outside of the clinic. The PCC facilitates administrative/non-medical operations in the clinic.
DUTIES AND RESPONSIBILITIES:
Schedule patient appointments both on and off the clinic site. Provide patient records to the referral source and advise the patient of the details of the follow-up visit.
Maintain follow-up log to track patient referrals, obtain reports, and requested medical records.
Co-Manage procuring branded medication through the Patient Assistance Program.
Responsible for screening process including training new volunteers to obtain proof of income and residency to determine patient eligibility for clinic services and PAP.
Respond to patient/telephone inquiries regarding clinic hours and services.
Track all inventory and orders supplies as necessary.
Responsible for entering new patient demographic information in Athena medical record.
Provide channel of communication for Spanish and/or Portuguese speaking patients and medical interpretation when needed.
Acknowledge that AFC maintains a policy and procedure manual at all locations and agree to follow the policies and procedures set forth within the manual.
Perform other duties as may be requested by Americares.

Qualifications:
Prior Medical Office/Clinic administrative experience a plus.
Minimum of Associates Degree (A.A.S.) preferred.
Strong interpersonal skills and ability to work well with medical staff, volunteers and patients.
Bilingual (Spanish) or multilingual required.
Must prioritize workflow and work independently yet respond to requests from others as needed.
Must participate well in a busy, team setting.
Expertise in use of electronic health records, in particular Athena, a plus.
Familiarity with Windows based software and ability to learn and subsequently train others on RxAssist and Athena electronic health record.
Compliance with Americares Free Clinics safe work policy as it pertains to Covid-19
