What are the responsibilities and job description for the Nurse: Full-Time (RN/LPN) position at Amoskeag Health?
The team nurse acts as patient advocate by assuming responsibility for coordinating patient’s care. Duties include: nurse visits, triaging phone calls, walk-in patients. The Team Nurse position also coordinates the flow of patient communications regarding: lab work, specialty referrals, medication refills/dosages.
Some flexibility in hours available. Combination of onsite and remote work available after orientation and training period.
PRIMARY:
1. Assess patient's need for treatment or advice via telephone call or personal encounter (walk-in). Consults with provider whenever necessary in order to advise patient of most appropriate action; reinforce provider plan of care, schedule appointments appropriately.
2. Maintain Nurse Visit schedule: responsible for attending to scheduled nurse visits and documenting care in EMR.
3. Meet with providers on a regular basis and work jointly to establish expectations for how team issues will be managed, such as scheduling, forms and refill issues.
4. Integrate health teaching into daily clinical routines, including appropriate information on health maintenance and disease management, for all ages, during each patient encounter.
5. In case of patient referral to the Emergency Department or another facility, coordinate care with appropriate staff at the other facility (Mental Health Center, Emergency Department, VNA, etc).
6. Prepare prescription refills and necessary vouchers, when indicated. Chart all refills and updates as needed.
7. Assist with the processing of patient paperwork. The nurse should prepare as many components as possible before handing off to the provider. Samples of patient paperwork are disability forms, handicap sticker requests, authorizations for wheelchairs and other assistive devices, letters to return to work, PE and Immune report requests. This list is not all inclusive.
8. Assist with non-scheduled nursing needs during/throughout the shift.
SECONDARY:
9. Promote patient confidentiality and right to privacy, cooperating with medical records staff to ensure effective retrieval system for client records.
10. Continually update personal, clinical and professional knowledge and skills through formal and informal educational experience. Comply with state continuing education laws.
11. Actively participate in policy/protocol development for the clinical setting.
12. All nurses are responsible to maintain their office space and to clean up after themselves if they have used an exam room, any medical equipment or instruments. Nurses will assist in pulling out full sharps dispensers and will replace those and other stock as needed.
13. Opening and Closing duties, Lunch coverage: There are duties responsible to the particular nurse who is covering at various times during the day. The specific duties are listed in our protocols. Each nurse is responsible for specific duties based on the shift they are working that day.
14. Become aware of and currently updated regarding community resources that would assist our patients.
15. Attend and participates in Amoskeag Health meetings as directed. If absent, responsible for reading minutes of meetings and signing the documentation.
16. Attend/complete all mandatory Health Stream, Compliance, safety-related, and infection control in-services/training as designated by the Senior Management Team.
17. Maintain licensure for RN/LPN status by ongoing professional education; maintain professional development.
18. Sensitivity to cultural diversity of population being served. Maintain patient age-related competence.
19. Other responsibilities as assigned by manager.
20. Nurse must be able to tolerate latex exposure from vaccine vials as well as tolerate perfumed oils, and other perfumed products.
Requires the ability to plan and perform a wide variety of duties requiring general knowledge of policies and procedures. Requires considerable judgment to work independently toward general results, devising methods, modifying or adapting standard procedures to meet different conditions, making decisions based on precedent and policy.
QUALIFICATIONS:
Education/Training: RN or LPN. Current NH RN or LPN license.
Experience: Community health experience preferred
BLS Certification Required
Knowledge/Skills: Ability to delegate tasks to other team members. Health assessment skills and coordination and delegation skills necessary.
