Aegis Home Health and Hospice is currently seeking a compassionate Medical Social Worker (MSW) that has a passion for providing outstanding services to the elderly in Senior Living Communities and in-home health settings. This is a full-time position. The two offices are located in Anchorage and Wasilla.

Job Responsibilities
- Provide short-term individual counseling
- Assess the psychosocial status of patient/caregiver related to terminal illness and communicate findings to the RN and other members of the interdisciplinary group
- Complete social evaluations including family dynamics, caregiver abilities, communication patterns, risk for suicide, neglect or abuse, and plan intervention based on evaluation findings
- Address patient/family questions and issues
- Educate patient/family on and assist in preparation of advanced directives
- Maintain a collaborative relationship with personnel to support patient care
- Serve as a liaison between patient/caregiver and community agencies
- Maintain clinical records for all patients
- Active participation in team meetings
- Demonstrate flexibility with job duties and assignments
- Master's degree from a school of social work (MSW) accredited by the council of social work education
- Minimum of one year experience in a health care setting; home care experience preferred
- Understand hospice philosophy, and issues of death/dying
- Strong communication and interpersonal skills
- Adaptive problem-solving skills
- Compassionate and caring attitude
- Valid driver's license, clean driving record, and reliable automobile
- Proof of current auto insurance
- Criminal background check clearance/fingerprint clearance
- Current CPR certification
- Negative TB test
- Works indoors in Agency office and patient homes and travels to/from patient homes.
Salary : $40 - $45
