The Medical Case Manager is a part of the Positive Connections team providing medical case management to those with HIV. The MCM is responsible for conducting a medical case management intake and needs assessment to formulate a care plan with the ultimate goal of improving the client/patient medical outcome. Medical case management experience preferred. Looking for positive, energetic, independent individual with organizational & computer skills. Pay-salary per nursing pay scale. For this position in Fayetteville, the MCM also performs clinic duties on HIV clinic days. The clinic nurse works as part of the clinic team and assists the provider in the provision of primary and HIV health care. 40-hour work week. Travel is required. Qualifications: Current state nursing license and CPR.
