What are the responsibilities and job description for the Registered Nurse (PRN) position at Arden Hospice?
- The PRN nurse will be available by phone to meet the needs of the patients, as assigned or as needs arise, physically making visits to patients after hours.
- Responsible for all communication related to on call activity documentation and communication to the Case Manager with assessment, plan of care changes and on-going need.
- Participate in morning stand up meetings to report off and stand down visits to receive reports
- Responding to calls from answering service and voice mail system in a timely manner.
- Complete changes to the plan of care as needed with IDG collaboration.
- Document all calls, visits, care plan changes, physician contacts, update orders and MAR consistent with policy and procedure.
- Coordinating services with contracted vendors as needed.
- Respond to efficiently and informatively referrals calls and information visits.
- Perform after hour patient admission visits as assigned by supervisor, death visits and all visits to address patient needs.
- respond to all patients/caregivers respectfully and calmly to address all needs or concerns.
- participate in IDG meetings
