We are looking for a dynamic and passionate Core Measures Specialist to join the Mountainside Team!
What You’ll Do
The Core Measures Specialist will access the clinical care and outcomes related to Quality Reporting, which includes concurrent management of clinical care for Core Measures and abstraction of medical records, reporting of the data to appropriate governmental and regulatory agencies (and other agencies as necessary), and ensuring the goal of reaching regulatory and system bench marks.
The successful candidate will be able to:
- Perform concurrent and medical record review, analysis and reporting for Core Measures/ORYX indicators, as well as participation in other Quality Reporting Programs
- Work collaboratively with other Core Measures Specialists, Staff Nurses, Nursing Leadership, and physicians to ensure compliance with all Quality Measures
- Facilitate team meetings and attends departmental meetings as necessary
- Work collaboratively with medical records and IT departments for the purpose of data abstraction and integrity
- Perform concurrent and/or medical record review and data collection for Core Measure/ORYX indicators including but not limited to Stroke and Sepsis
- Maintain log and ensure charts are complete and sent out as requested
- Serve as a point of contact for the hospital for information regarding Q Net
- Confirm the accuracy of all population and sampling data as well as the number of Core Measure cases uploaded to Q Net via our software vendor by running reports via the Secure Q Net Site
- Orientation/education to new Core Measures Specialists
- Coordinate the Stroke Program
- Act as team leader for The Joint Commission Disease Specific Certification program for Stroke
- Maintain current knowledge of Quality Reporting via Q Net, as well as participation in all webinars
- Perform any and all other duties assigned
What You’ll Bring
- Bachelor’s degree in healthcare-related field required, RN strongly preferred; health education preferred
- At least 3 years of hospital experience
- Strong foundation of clinical knowledge
- Ability to analyze, interpret and share data
- Excellent communication and teamwork skills
- Strong organizational skills
- Ability to work independently
- Proficient in Microsoft Office Products, Vendor software systems and Electronic Health Records
