What are the responsibilities and job description for the Prevention Navigator position at Argus Community, Inc.?
Job Summary:
The Prevention Navigator provides referrals to NYS OASAS-certified programs, 30-60 day intensive behavioral health programs and housing stabilization support for in-crisis persons at risk for substance misuse who are HIV or at risk for HIV. Prevention Navigator will also provide prevention navigation services for HIV and at risk population and will provide referrals for food stability, STI testing and prevention, medical care and medication adherence.
Primary Duties & Responsibilities include, but are not limited to:
- PN will provide prevention services for HIV and high-risk clients who are in crisis due to substance misuse, co-occurring mental disorders, and unstable housing with access to comprehensive services for indicated medical care, behavioral health care, stable housing, and other support services while maintaining compliance with confidentiality regulations including NYS OASAS 42 CFR.
- Support client education on safer injection and sexual behaviors, Engagement and retention in confirmatory testing and treatment for HCV, HAV, HBV.
- PN facilitates communication between client and health care professionals, assists with issues of health literacy, provide placement and warm handoffs for behavioral health services, provide client escort and transportation assistance when necessary, overcome barriers to care including lack of trust in providers, improve service coordination, assure accurate information across multiple providers, provide assistance with legal needs and immigration issues, provide access to resources for food insecurity, and support clients to develop self-navigation and advocacy skills.
- Use culturally competent practices to work with care management teams of programs with which CATCH/EIS has linkage so that crises are resolved, clients are stable participants in treatment plans and are on the path to treatment adherence and recovery.
- Provide follow-up at 60 days to document client sustained engagement in treatment.
- Meet monthly with a senior Argus clinical case supervisor for clinical and administrative supervision of the PN program.
- PN data aggregation and reporting, and analysis of PN data to evaluate service outcomes.
- Maintain timely AWARDS database records that delineate involvement in the service and are maintained in accordance with applicable state and federal confidentiality law.
- Attend all required staff meetings and trainings.
Qualifications/Criteria:
- Minimum of 2 years’ experience with persons affected by HIV SUD/COD, and/or homelessness with some leadership experience.
- Must be able to perform fieldwork in the Bronx, Manhattan, Brooklyn, and Queens.
- Must be available to work 35 hours per week Monday to Friday.
- Bilingual English/Spanish preferred.
Education:
- High School Diploma or GED.
