Job Summary:
The Lead Regional Resource Development Specialist (Lead RRDS/NHTD) reports to the Assistant Director of Long Term Care. The Lead RRDS must be an experienced professional with demonstrated skills and a dedicated commitment to working to support seniors and individuals with disabilities who wish to live in community-based settings.
Qualifications:
Bachelor’s Degree or higher in health or human services or a related field and four years of combined satisfactory professional experience in working with individuals with disabilities and/or seniors
Or
Registered Professional Nurse and four years of combined satisfactory professional experience working with individuals with disabilities and/or seniors, including in a managerial capacity
Or
Associate’s Degree and 10 years of combined satisfactory experience working with individuals with disabilities and/or seniors, including in a managerial capacity
Or
Associate’s Degree and four years of satisfactory experience as a Regional Resource Development Specialist
Must have at least one year of experience working at a Regional Resource Development Center
Must have experience supervising professional staff who work directly with seniors and individuals with disabilities. Managerial capacity includes, but is not limited to: program budgeting, staff scheduling and work assignments, staff performance evaluations, direct supervision of workers, program development and planning, program quality assurance and program operations.
Must have knowledge of the operation of 1915 (c) waivers, NYSDOH policies and procedures, and guidelines/regulations and community-based services, as well as working knowledge of services and supports available through Medicaid, Medicare and/or other third party payers
Clean driver license, reliable transportation
Essential Functions:
Manage and deliver the program objectives of conversion, repatriation, diversion, waiver administration and resource development
Implement the HCBS/NHTD waiver and improve access to NHTD program supports
Assess, identify, and address gaps in services
Develop and implement a comprehensive outreach campaign to recruit Wavier providers
Build NHTD community consortiums and develop NHTD provider competence
Develop relationships with Local Department of Social Services and other local and state entities, community-based healthcare providers, and inpatient rehabilitation units and long term care facilities
Meet with potential waiver participants, determine waiver eligibility, and offer individuals the choice of participation in the HCBS/NHTD waiver and a choice of waiver Service Coordinators
Review, monitor, and approve HCBS/NHTD waiver participant Service Plans that are developed by the waiver participant and the Service Coordinator, and review revised plans annually to monitor for quality and cost effectiveness
Interview potential waiver providers and make recommendations to Department of Health (DOH) on whether to enroll them as providers based on factors such as regional need or the provider’s ability to deliver quality, cost-effective services to the NHTD population
Provide training and technical assistance to waiver service providers on the needs of the individuals with disabilities and how to develop waiver Service Plans
Review all information and investigations regarding Serious Reportable Incidents (SRI) in accordance with the SRI policy, collaborate with DOH as needed, and provide technical assistance and training to providers on the SRI process as needed
Provide technical assistance to participants and family members by providing information, answering questions, and addressing concerns as needed
Collaborate with the Open Doors transition center and complete the bi-annual report to DOH
Develop and submit monthly and quarterly reports to DOH
Work closely with DOH to provide data and input as needed regarding all aspects of the administration of the HCBS/NHTD waiver in the region
Participate in agency, departmental, and community meetings
Implement ARISE’s compliance program and the Employee Code of Conduct as required
Uphold policies and procedures
Exercise sound judgment as a matter of course and hold protected health information and other sensitive information in the strictest of confidence in accordance with ARISE and HIPAA policies
