Job Summary:
We are seeking a skilled and compassionate Licensed Physical Therapist (PT) to join our team. We schedule 30-minute appointments that are never double booked to increase 1:1 time spent with patients, allowing you to focus on patient care and point of service documentation. As a key member of our integrated healthcare approach, you will play a crucial role in enhancing the overall well-being of our patients by providing high quality physical therapy services.
Supervisory Responsibilities:
Position does not have direct reports but will work closely with team members in providing patient care.
Job Responsibilities:
- Conduct thorough assessments to evaluate patients' physical conditions and establish personalized treatment plans.
- Administer evidence-based physical therapy interventions to address musculoskeletal and neurological conditions contributing to pain.
- Collaborate closely with nurse practitioners, chiropractors, and doctors to create integrated care plans tailored to each patient's unique needs.
- Educate patients on therapeutic exercises and home care routines to support their rehabilitation and pain management goals.
- Communicate effectively with the healthcare team to ensure seamless coordination of care and a holistic approach to patient treatment.
- Document patient evaluations, progress notes, and treatment plans accurately and in a timely manner.
- Stay informed about advancements in physical therapy practices and incorporate new techniques into patient care when appropriate.
- Participate in team meetings and professional development activities to enhance collaborative efforts within the clinic.
- Uphold ethical standards and maintain licensure requirements, demonstrating a commitment to high-quality patient care.
- Requires the effective use of office equipment including, but not limited to, computers, phones, and printers.
- Ability to sit at computer terminal for extended periods of time.
- Ability to communicate via telephone.
- Ability to communicate in an active office environment.
- Must be able to lift to 15 pounds at times.

Education and Experience:
- Valid and active license to practice as a Physical Therapist in Arizona.
- Previous experience in a multidisciplinary healthcare setting is preferred.
