How You'll Make a Difference:
Arkos Health is seeking a Clinical Services Manager. The Clinical Services Manager will be responsible for shaping and implementing our population health strategies and programs. You will lead a team of dedicated nurse care managers and collaborate with cross-functional teams to ensure the effective management and coordination of care for our patient population. Your expertise will be instrumental in driving positive health outcomes and achieving our company's mission.
Essential Duties and Responsibilities:
- Care Management Leadership: Provide strategic direction and leadership for the population health care management team, ensuring the delivery of high-quality, evidence-based care
- Team Management: Recruit, train, and manage a team of care managers, nurses, and support staff to ensure they are well-equipped to provide exceptional care coordination and support
- Program Development: Collaborate with the executive team to design, implement, and optimize population health programs, including care plans, care transitions, and disease management initiatives
- Clinical Oversight: Review and assess the clinical quality of care management activities, ensuring compliance with industry standards and best practices
- Data Analysis: Utilize healthcare data and analytics to identify trends, gaps in care, and opportunities for improvement in population health outcomes
- Patient Engagement: Develop strategies to engage and empower patients in their healthcare journey, promoting self-management and adherence to care plans
- Interdisciplinary Collaboration: Foster collaboration among care managers, physicians, social workers, and other healthcare professionals to ensure seamless care coordination
- Performance Metrics: Establish and monitor performance metrics and key performance indicators (KPIs) to track the success of care management programs
- Compliance: Ensure that all care management activities adhere to regulatory requirements and company policies
- Quality Improvement: Lead continuous quality improvement efforts, implementing changes based on data-driven insights to enhance patient outcomes
Qualifications and Skills:
- Current, unencumbered, Registered Nurse License and graduation from an accredited Registered Nurse program
- Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing or related field preferred
- Previous experience in care management, population health, or related roles
- Strong leadership and team management skills
- Proficiency in healthcare data analysis and reporting
- Knowledge of healthcare regulations and compliance
- Commitment to patient-centered care and improving health outcomes
- Language & Communication Skills: Must communicate well with all levels of medical professionals and possess knowledge of acute care, medications /treatment.
- Must be fluent in both spoken and written English and maintain professionalism in all verbal and written correspondence with both internal and external clients
- Previous experience with Medicaid/Medicaid populations preferred
