We are currently seeking an experienced RN Case Manager to join our team. The successful candidate will be responsible for managing and coordinating the care of patients with complex medical conditions. The RN Case Manager will work closely with patients, families, physicians, and other healthcare professionals to develop and implement a comprehensive care plan.
Requirements:
- Current RN license in the state of Indiana
- Minimum of 2 years of experience in a Acute care Hospital setting, preferably in case management or care coordination
- Bachelor's degree in nursing preferred
