Position Description:
The Intake Coordinator provides direct counseling and case management services to patients, consisting of individual, and group services, including assessment, individualized treatment plans, counseling and aftercare services. The Intake Coordinator will adhere to the vision and mission of the Company.
Duties and Responsibilities:
- Conducts intake screeners in conjunction with call center
- Schedule weekly admissions through electronic scheduler
- Responsible for reviewing pre admission paperwork.
- Conduct clinical admissions.
- Complete Interpretive Summaries within 7 days of admission.
- Complete Comprehensive treatment plans for all new admits.
- Be proficient with State Licensure Standards, CARF Standards, 42 CFR and HIPAA standards and Company policies.
Qualifications:
- Bachelors Degree in psychology, counseling or human services
- Minimum of one-year experience, preferably in a substance abuse environment
- Proficient in the use of personal computers
- Experience in using NJSAMS
- Experience in community outreach/ census development preferred
- Must be willing to travel within 90 miles of both Mays Landing and Cape May Court House
