Join Our Team! Social Workers are an integral part of our multil-disciplinary care team at Lourdes. You will serve as an essential resource to our Hospice patients, their families and care team utilizing your exceptional assessment and plan interventions experience. As a Hospice Social Worker, your distinct role will include advocating for each patient's end-of-life wishes and helping individuals address the emotional aspects of late-stage illnesses. The ideal candidate will have previous work experience supporting others through the death and dying experience, demonstrate the ability to work autonomously and be accountable to meet the needs of our Hospice patients and their loved ones.

What You Will Do
Investigate, assess and plans interventions to help patients cope with social, emotional, economic and environmental problems.
- Interview patients, assess priorities and document case activity.
- Consult with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
- Assist clients/families with obtaining community assistance by referral to proper resources. Develop and maintain working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
- Assure resident safety by identifying safety hazards and emergency situations and initiates corrective action as appropriate.
What You Will Need
Licensure / Certification / Registration:
- Social Worker credentialed from the New York Board of Social Work obtained within 2 Weeks (14 days) of hire date or job transfer date. SW OR LMSW is required.
Education:
- MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE) required
Work Experience:
- One year of experience in a health care setting required.
