We Are Hiring

Lourdes, part of Ascension in Binghamton, NY, has an exciting opportunity for a full-time LPN-Patient Navigator to join our Urology team located at 169 Riverside Drive in Binghamton.  In this role you will guide patients through the healthcare system and help them overcome barriers to optimal care.  Candidates with Urology, oncology, and surgical experience with high attention to detail and excellent communication skills are strongly preferred.  If you enjoy working in a collaborative, multidisciplinary, compassionate team and have at least 2 years of experience working as a LPN this position might be a great fit for you.

Schedule:  Monday - Friday: 8:30 am to 5:00 pm


What You Will Do

Serve as a patient navigator to accommodate patients throughout the continuum of care.

  • Coordinate and lead presentations to prospective patients which may include community seminars, expositions and support groups.
  • Provide support with navigating the healthcare system and assist patients with utilizing available community support services.
  • Address barriers to care.
  • Work with Navigation Team as needed and serve on hospital committees as assigned.
  • Work with physicians and the health care team to accommodate patient issues and to ensure quality patient care.

What You Will Need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
  • Practical/Vocational Nurse credentialed from the New York Board of Nursing obtained prior to hire date or job transfer date required.

Education:

  • High school diploma or GED required.

Additional Preferences

At least 2 years of LPN experience strongly preferred.
