What are the responsibilities and job description for the RN-Patient Navigator position at Ascension?
Join our Ascension Providence Team at our Southfield and Novi Campus as an RN - Patient Navigator.
This is a full time position. Monday through Friday, 8 hour days (7 am -3: 30 PM or 8 AM -4:30 or 9:00 AM -5: 30 or what fits the need of that day). No Weekends or holidays.

What You Will Do
Serve as a patient navigator throughout the continuum of care.
- Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
- Coordinate efforts for treatment and promotes quality improvement and educational efforts.
- Compile data, tracks outcomes, and make recommendations for process improvement.
- Provide referrals/support for follow-up care for patients who are uninsured or underinsured.
What You Will Need
Licensure / Certification / Registration:
- Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider credentialed from the American Heart Association (AHA) preferred.
Education:
- Diploma from an accredited school/college of nursing required.
Additional Preferences
- Acute care and/or oncology nursing experience,
- Excellent organization and communication skills, compassionate, and forward thinking.
