POSITION: Comprehensive Prenatal Health Worker (CPHW)/ Medical Interpreter (MI) I - Cambodian Speaking  

OFFICIAL DUTY STATION: Los Feliz Health Center (1530 Hillhurst Ave. Los Angeles, CA 90027)

STATUS:  Full Time FTE; Non-Exempt (0.60 CPHW and 0.40 Medical Interpreter I)

REPORTS TO: Maternal and Child Health (MCH) Unit Manager, Clinic Operation  Manager   

SUPERVISES: None

SUMMARY:

Comprehensive Prenatal Health Worker (CPHW) shall provide high quality and customer friendly Comprehensive Perinatal Services Program (CPSP) to qualified patients, and meet standards and quality and comply with external regulations, program and requirements.

CPHW assures the delivery of primary care services by preparing and providing timely availability of CPSP services for APHCV patients at Los Feliz Health Center.

CPHW is responsible for meeting the standard and productivity expectations, following policies and procedures (P and Ps) and service standard, curriculum, and work flows. CPHW shall attend training to keep updated and demonstrate competency in all aspects of CPSP program. CPHW shall submit required performance report to MCH Unit Manager, as well as Medical Director and TQM Committee.

CPHW utilizes interview format to conduct prenatal and medical histories, and educational, psychosocial and nutritional assessments following established policies and procedures.

CPHW provides the “support services” ongoing orientations, assessments, care plan development, appropriate nutrition, health education and psychosocial interventions and referral when needed to our prenatal patients. CPHW also provides support and health information as indicated by the assessments as well as referrals to community agencies and resources. Identifies high risk patients and refers according to established policies and procedure of APHCV.

CPHW maintains a close working relationship with Provider, Nursing staff and other clinic staff to support service delivery and effective CPSP service operations.

As APHCV is in the business to serve patients APHCV is committed to the prevention of patient centric care at all level, from the CO Call Center Operator, Front Office and Medical Interpreters (and beyond). This position helps facilitate successful delivery of services to linguistically diverse patient consumers by providing interpretation for medical services and non medical services such as enrollment and other benefit programs.

Comprehensive Prenatal Health Worker (0.60 FTE)

DUTIES AND RESPONSIBILITIES:

  1. Meet the CPSP productivity and quality standard goal and submit productivity and quality standard reports to the MCH Unit Manager.
  2. Provide CPSP, Prenatal Orientation and case coordination.
  3. Conduct face-to-face (telephone currently due to COVID19 pandemic) interviews with all assigned Bengali and English speaking prenatal patients (unless assigned differently by the MCH Unit Manager), screens patients for presumptive eligibility (PE) and issuing of PE if eligible.
  4. Provide direct patient services through assisting registration, history intake, and making appointments and providing CPSP Orientation and initial, follow-up, and postpartum assessment.
  5. Provide ongoing orientations (for example, orientation to CPSP program, providers, delivery hospitals, WIC program, etc.), assessments, and classes and makes the appropriate referrals when needed to our prenatal patients.
  6. Provide documentation in the patient’s medical chart and develops the patient’s Individual Care Plan (ICP) after the initial prenatal history.
  7. Monitor patient’s attendance to prenatal, RD, LCSW and Pediatric appointments, contacts patients to reschedule any missed appointments and documents in the patient’s medical charts any contact made with patient.
  8. Provide warm hand off when possible or appointment for patient to meet with Enrollment staff. Enrollment staff will advocate for patients with the Medi-cal worker.
  9. Act as a resource to the patient for problems and questions. Assists the patient in problem solving during and after pregnancy.
  10. Maintain patient files and completes data entry on each patient after each visit or contact including telephone calls to ensure that all documents are completed as required by the CPSP program procedures. Signs all chart entries using legal signature and title.
  11. Conduct patient trimesters and postpartum assessments, completes assessment forms and updates care plan following CPSP guidelines.
  12. Make referrals to appropriate community agencies and organizations which also include CPSP Mandated Services (i.e. WIC, Genetic Screening, Family Planning or CHDP) to meet patient’s needs according to APHCV’s policies and procedures. Agencies organizations shall be those that are approved by APHCV. 
  13. Provide case management service or case coordination after reviewing all prenatal charts after each clinic visit with the provider, as well as reviewing each chart to assure that patient has received necessary services based on the guideline (CPSP Step to Take Guideline).
  14. Schedule mother’s four week postpartum and infant’s newborn check-up appointment at the health center.
  15. Conduct hospital visits and does bedside teaching on postpartum and newborn care. (TBD, transitioning to eventually)
  16. Counsel patients on all methods of birth control and charts in patients’ medical chart any counseling done. Assist in FPACT application if needed.
  17. Interact with medical providers, nursing staff, nutritionist, and other team members for interdisciplinary team approach to patient’s care.
  18. Follow the registration and check-in workflow via Curago Health portal as prescribed by the IT and APHCV’s Clinic Managers.
  19. Adhere and follow the CPSP Visits via Telephone/Telehealth during COVID19 Pandemic Procedure.
  20. Provide the completion of the patient encounter form for services provided.
  21. Prepare all Intake packets in English and makes sure all appointments have been given to patient accordingly, i.e. Intake, Initial, Medi-cal, etc.
  22. Assure that all procedures and tests that the Provider has ordered have been done and lab results have been received on a timely manner as highlighted on the Prenatal Program Checklist.
  23. Attend recommended job related trainings provided by the CPSP County program.
  24. Ensure that prenatal records are transferred to the hospital’s Labor and Delivery per the Provider’s instructions.
  1. Ensure that newborns are scheduled for their initial pediatric appointment within 3-5 days of delivery or within 72 hours of hospital discharge.
  2. Assist the MCH Unit Manager in achieving goals.
  3.  Participate in data collection and reporting for each patient served and ensures that all required data is provided on each patient at specific required intervals. All forms completed and signed and all entries completed in a timely manner as described by the program.

 

A. Expectations for CPHW:

1. Client Load Expectation: Maintain at least  25-35 CPSP patients/per full time CPHW at any given time to start out with. This expectation will be re-evaluated in an on-going basis.

B. Quality Management: 1. Display knowledge of normal signs of human development and ability to assess and provide prenatal patient age and gestational age appropriate care. 2. Contribute to the success of the organization by participating in quality improvement activities.

C. Customer Relations: 1. Respond promptly and with caring actions to patients and employees. Acknowledge psychosocial, spiritual and cultural beliefs and honor these beliefs. 2. Maintain professional working relationships with all levels of staff, clients, and the public. 3. Be part of a team and cooperate in accomplishing department goals and objectives

D. Safety: 1. Maintain current knowledge of policies and procedures as they relate to safe work practices. 2. Follow all safety procedures and report unsafe conditions. 3. Use appropriate body mechanics to ensure an injury free environment. 4. be familiar with location of nearest fire extinguisher and emergency exits. 5. Follow all infection control procedures including blood-borne pathogen protocols

E. HIPAA/Compliance: 1. Maintain privacy of all patients, employees and volunteer information and access such information only on a need to know basis for business purposes. 2. Comply with all regulations regarding corporate integrity and security obligations. Reports all behaviors and/or activities that are unethical, fraudulent, or unlawful to supervisor immediately.

 

Medical Interpreter I (0.40 FTE)

 

DUTIES AND RESPONSIBILITIES:

Patient Support Service:

  1. Provide intake and assist in financial screening and Health History intake for limited English proficient patients.
  2. Provide interpretation at various points of patient interaction to support clinic operations such as scheduled patient visits with providers and patients.
  3. Call patients 2 days before their appointment to remind them of their appointment.
  4. Provide either interpretation or instruction at classes for patients on various health topics.
  5. Retrieve telephone messages and contact patients in a timely manner in response to voicemails left by patients.
  6. Contact and follow up with patients who no-showed for their medical appointment(s) in accordance with the “No Show Follow Up” procedure. Document such patient contact in EPM or EHR (Telephone Template).
  7. Forward patient’s concern and feedback to appropriate personal within APHCV for resolution and optimization of clinic efficiency.
  8. Work with clinic operation and medical staff to facilitate efficient patient flow.
  9. Participate in data collection and verify advanced access data as a special project as well as regular day to day activity.
  10. Assist with data collection for clinic performance measures.
  11. Call patients with abnormal lab results, etc (tasked by triage nurse, providers, and etc.). Document such calls made in NG EHR (Telephone Template).
  12. Schedule future appointments, reschedule or cancel appointments when necessary.
  13. Submit quality of care and productivity reports to Supervisor, if any.
  14. Task triage nurses and referral nurses and also respond to the various tasks accordingly.
  15. Change and update primary care providers for patients according to the assignment.
  16. Create and deactivate the alert in NG EPM.
  17. Performs other duties as assigned by supervisor/Clinic Operations Manager.

 

Translation:

  1. Translate vital documents (clinic documents, outreach materials, health education materials and etc.) in timely manner.
  2. Perform back translation or proofread the translation to evaluate the accuracy of translation
  3. Type set or coordinates with other staff to type set the translated material.

 

Professional development/others:

  1. Attend APHCV staff meeting, clinic staff meeting, and other meetings as assigned by supervisor.
  2. Attend trainings as assigned by supervisor/clinic operations manager.
  3. Communicate training needs to supervisor/clinic operations manager.
  4. Participate in agency wide event
  5. Attend a continuing education class for furthering and keeping abreast in the medical interpretation field.

 

PERFORMANCE EXPECTATIONS:

  1. Maintains professional and excellent customer service demeanor at all times with patients caregivers, and other staff.
  2. Works as a team player and be willing to assist co-workers in other duties within his/her capabilities, to aid the clinic team in providing good efficient patient care.
  3. Facilitate patient flow by working effectively and efficiently.
  4. Maintain open communication with other clinic staff.


Qualifications for CPHW

  1. At least 18 years old
  2. High school diploma or equivalent
  3. One year full time paid prenatal experience.
  4. Knowledge and familiarity with clinical setting preferred

Qualifications for Medical Interpreter I

  1. 1 year minimum of substantial volunteer or working experience in the targeted community.
  2. 1 year interpretation/translation experience.
  3. Ability to speak, read, and writes fluently in English and targeted language.

 

SKILLS PREFERRED:

  1. Proficient in PC/MAC with working knowledge of Microsoft Office applications.
  2. Experience with intranet and internet.
  3. Computer literacy in English and targeted language.
  4. Basic knowledge of prenatal (OB) care and health in general.
  5. Knowledge of targeted communities as a Medical Interpreter.
  6. Strong communication skills.
  7. Regular predictable attendance during clinic hours.
  8. Ability to work efficiently, accurately, and with minimum supervision.
  9. Ability to follow directions and follow up on recommendations from supervisor.
  10. Ability to work with people of diverse cultural, educational, socio-economic, and linguistic backgrounds.
  11. Knowledge of health care system in the United States.
  12. Knowledge of health promotion and education.
  13. Working experience in medical field, case management, and customer service.
  14. Ability to teach or instruct others.
  15. Interest in community health.
  16. Detail oriented.
  17. Skilled in time management with multiple priorities.

 

HR PROCEDURAL REQUIREMENTS:

  • Legal authorization to work in the United States
  • A valid California Driver’s license with clean records and access to insured automobile
  • Completion of APHCV Health Assessment Form
  • Completion of DOJ background check

 

PHYSICAL REQUIREMENTS:        

Must be able to materially perform the task normally associated with the position including but not limited to: ability to lift up to 25 lbs.
