What are the responsibilities and job description for the Care Navigator position at Aspire Health Partners?
Job Purpose:
As a Care Navigator at Aspire Health Partners, your job purpose to provide overall assessment and needs of chronically mental ill adults. These functions include – but are not limited to – providing screening, referral, emergency, crisis and linkage services, participating in clinical treatment team, documenting clinical records, and additional program specific documents while maintaining the integrity of the client, understanding and ability of the necessary technology to communicate effectively with staff. Staff will be responsible for linking clients to appropriate community resources and programs to meet unique needs. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures. Staff will be co-located at Central Florida Behavioral Hospital.
