Description:
The Patient Service Coordinator is responsible for coordinating the lifetime care of the patient as well as being a dedicated resource for the physician and the care staff both at the at-home and facility sites of care. They work with the patient and their care team throughout the patient's journey. This includes monthly coordination of medication for the patient, contacting prescribers as needed, and working with the reimbursement team to ensure the patient fully understand the impact to their benefits.
Duties and Responsibilities:
-Ensure a positive patient experience throughout the entire patient journey
-Manage the active patient case to include coordination of drug shipment and supply processing by coordinating refills and ensuring ongoing patient compliance with medical regimen.
-Own the relationship with all care partners such as the patients' physician, nurse, caregivers, and pharmacist.
-Maintain confidentiality and integrity of protected health information and follow HIPPA security policies and procedures.

Must have open availability from 7am-7pm M-F
