At Home Harmony is transforming how healthcare is delivered to aging seniors and medically vulnerable members in our community. Our private practice is focused on keeping seniors healthy by providing care in their homes through an interdisciplinary team of providers, pharmacists, nurses, social workers and others – all working together to meet our patients’ needs.
Our Care Management (Chronic Care Management (CCM)/Remote Patient Monitoring (RPM)) Services are a key component of caring for this patient population. Through this program, we support patients with complex medical conditions with a goal of improving care coordination, patient outcomes and patient satisfaction. We also manage chronic conditions through our Remote Patient Monitoring (RPM) program which requires patient monitoring and management. We are seeking a friendly, outgoing and tech savvy registered nurse with a positive attitude and strong work ethic to serve as our Care Management (CCM/RPM) Nurse. Proven dependability and ability to succeed in a fast-paced and ever-changing environment are most important.
Job Duties of the Care Management Nurse include:
· Collaborate with a team of providers and other support staff to identify patients who are eligible for and would benefit from CCM and RPM
· Assist providers in educating patients regarding CCM and RPM benefits, patient responsibilities, patient consent, monthly appointment commitments, etc.
· Complete initial patient intake and needs assessment in collaboration with the healthcare team and facilitate therapeutic communication with patients and families to maximize coordination of care.
· Develop care coordination plans and goals mutually agreed upon by patient, family, and provider that meet CCM guidelines.
· Complete regularly scheduled visits (via phone or in person) to qualified patients.
· Provide triage services to patients of At Home Harmony.
· Manage panel of patients on RPM and collaborate with providers and support staff regarding RPM reports and needs.
· Provide education regarding disease management risk reduction, strategies and tactics.
· Complete clear, concise, and timely documentation in the EHR and supporting software to meet CCM/RPM documentation requirements.
Qualifications and Requirements:
· Registered Nurse in good standing in Virginia
· 5 years’ experience in at least one of the following settings: case management, disease management, home health care, hospital, or physician office setting and
· 2 years’ electronic medical record (EMR) experience – Athena EMR and/or Prevounce software, ideal
· Knowledge of chronic care monitoring – preferred
· Strong computer skills
· Strong communication skills
· Outgoing and friendly personality
This practice offers benefits and works weekdays only between the hours of 8am-4pm – no evening or weekend hours required. This position will be based out of the practice’s Richmond office with opportunity for hybrid/remote work.
