We seek a motivated and experienced Full-Time TBI/NHTD Service Coordinator (Medicaid Waiver) for the Binghamton, NY, area. The Service Coordinator assists prospective waiver participants and coordinates and monitors the provision of all services for the Traumatic Brain Injury (TBI) Waiver and the Nursing Home Transition and Diversion (NHTD) Waiver. The Service Coordinator's goal is to increase the participant's independence, productivity, and integration into the community while maintaining the health and welfare of the individual. The ideal candidate will have excellent social skills.
Duties:
- Provide direct care management services, maintaining accurate and verifiable documentation of all services rendered.
- Gather information needed for applicants to be approved as waiver participants by regional RRDC.
- Assist clients/ legal guardians with applications and paperwork for and management of benefits, financial, housing, or other related areas as needed.
- Develop comprehensive service plans, PPOs, and all other required DOH paperwork for participants assigned.
- Monitor implementation of each client's service plan and provide ongoing review of the plan's overall effectiveness in compliance with all state requirements and regulations.
- Maintain a minimum monthly face-to-face contact with each client served.
- Formally review and submit a revised service plan as the applicable Waiver directs.
- Maintain client records for a minimum of 7 years after termination of Waiver services.
- Ensure the assessment of the need for a nursing facility level of care is completed annually.
- Coordinate and facilitate team meetings with participants, their natural supports and all service providers semi-annually and as requested by clients receiving services from TBI/NHTD Waiver programs.
- Participate in staff meetings and other Service Coordination department activities and mandatory training.
- Ensure compliance with all applicable local, State, and Federal regulations and agency policies.
Requirements:
- Licensed Master Social Worker; or Licensed Clinical Social Worker; or Master of Social Work degree; or Master of Psychology degree; or Certified Teacher of Students with Disabilities; or Certified Rehabilitation Counselor and minimum one (1) year of experience providing case management/service coordination, information and referral services, and service brokering of community-based services for individuals with disabilities, TBI, and/or seniors.
OR
A Bachelor's degree in a related field and three (3) years of experience providing case management, information and referral services, and service brokering of community-based services for individuals with disabilities.
OR
Associate's degree in conjunction with at least five (5) years of experience in social services, providing case management and direct hands-on care.
- TBI/NHTD waiver or case management experience is preferred.
- Knowledge in completing/conducting an biopsychosocial assessment as well as current knowledge of supports within the community.
- COVID-19 vaccination required

**This is a remote position. However, it is required to visits your participants in their home quarterly (reimbursement will be provided for all work-related traveling expenses with proper receipts). There will also be an officemeetingevery 6 months at a designated location.

Pay: $50,000.00 - $60,000.00 per year
